SHA Forms
Following is a list of the various forms that may be required or requested by SHA from students and/or parents. If you select a form it will open, so you can then print the form, complete it, then scan and send it to SHA. You may also submit the paper version with your child.
- Elementary Student Supply List 2026-27
- Elementary Uniform Sweatshirt Order Form (KP-6)
- Health Appraisal (KP-K only, @ initial enrollment only)
- Secondary Student Supply List 2026-27
- Concussion Awareness Form (all students, only once)
- Dispensing of Medication: Release (must be completed annually)
- Community Service Form (grades 7-12)
- Permission for Non-SHA Student to Attend SHA Dance
- MHSAA Sports Physical Form (grades 6-12 must be completed annually by physician)
- Secondary Uniform Sweatshirt & 1/4 Zip Order Form (7-12)
