SHA Forms

Following is a list of the various forms that may be required or requested by SHA from students and/or parents. If you select a form it will open, so you can then print the form, complete it, then scan and send it to SHA. You may also submit the paper version with your child.

  1. Elementary Student Supply List 2022-23
  2. Elementary Uniform Sweatshirt Order Form (KP-6) 
  3. Health Appraisal (KP-K only, @ initial enrollment only)
  4. Secondary Student Supply List 2022-23
  5. Concussion Awareness Form (all students, only once)
  6. Dispensing of Medication: Release (must be completed annually)
  7. Community Service Form (grades 7-12) 
  8. Permission for Non-SHA Student to Attend SHA Dance
  9. Sports Physical Form (grade 7-12 must be completed annually by physician)
  10. MHSAA Health Questionnaire
  11. Secondary Uniform Sweatshirt & 1/4 Zip Order Form (7-12) 
  12. Elementary School Picture Form -  For pictures taken 8/31/22
  13. Elementary RETAKE Form - 10/25/22
  14. Secondary School Picture Form - For pictures taken 8/31/22
  15. Secondary RETAKE Form - 10/25/22
  16. Elementary Yearbook Order Form - Order Deadline March 15, 2023
