SHA Forms

Following is a list of the various forms that may be required or requested by SHA from students and/or parents. If you select a form it will open and you can then print the form, complete it then scan and send it to SHA or submit the paper version with your child.

  1. Elementary Sweatshirt Order Form
  2. Student Supply List 2016-17 (grades DK-6 only)
  3. Concussion Awareness Form (all students only once)
  4. Dispensing of Medication: Release (must be completed annually)
  5. Community Service Form (pdf) or Community Service Form (doc) (grades 7-12)
  6. Secondary (grades 7-12) Dress Code Sweatshirt Order Form
  7. Acceptable Use Policy for SHA Technology Systems (grades 7-12 completed annually)
  8. Permission for Non-SHA Student to Attend SHA Dance
  9. Sports Physical Form (grade 7-12 must be completed annually by physician)
