SHA Forms

Following is a list of the various forms that may be required or requested by SHA from students and/or parents. If you select a form it will open and you can then print the form, complete it then scan and send it to SHA or submit the paper version with your child.

  1. 2017-18 Elementary Sweatshirt Order Form
  2. Student Supply List 2017-18 (grades DK-6 only)
  3. Concussion Awareness Form (all students only once)
  4. Dispensing of Medication: Release (must be completed annually)
  5. Community Service Form (pdf) or Community Service Form (doc) (grades 7-12)
  6. 2017-18 Secondary (grades 7-12) Dress Code Sweatshirt Order Form
  7. Permission for Non-SHA Student to Attend SHA Dance
  8. Sports Physical Form (grade 7-12 must be completed annually by physician)
